**Responsible in supporting the care of the residents in the community and care providers, across the primary care and long-term care settings.**
- To work with community nurses to monitor residents with chronic disease in the community, through nodes like Community Nurse Posts and in the residents’ homes, where necessary.
- To provide basic nursing care activities and servicesto support residents identified from community and monitor frail patients discharged from hospital, to help them transit successfully from hospital to home with the aim of promote self-management and reduce re-admission rate.

**Job Scope**
1. Work in the community under the guidance of the community nurses in clinical care management, which include but not limited to:
a) Conduct home visits in thecommunity; Conduct basic parameters monitoring and basic general screenings e.g. eyes and falls basic screenings, during health screenings and health checks sessions in consultation with and/or refer to community nurses for further health/medicaladvices.

b) Perform basic physical assessments, observing, documenting and reporting clinical and treatment information, including patient's behavioralchanges to community nurses

2.Perform clinical care activities under the directive of a registered nurse

3.Provide education and co-management of the patients/residents with the community nurses

4. Maintain patients’ privacy and confidentiality of information and records at alltimes.

5. Demonstrate good interpersonal skill with public andstaff.

6. Participate in Multi-Discipline Meeting (MDM) to provide inputs and observations for discussion on patients’progress.

7. Assist in maintaining patients’ records and data entry.

**Requirement**:

- Secondary school education
- Preferably with WSQ Higher Certificate in Healthcare (Nursing Support) or equivalent
- Preferably with one year of relevant experience in home care, nursing home, community hospital / hospital setting or equivalent as Healthcare Assistant
- Possess positive attitude, initiative, and keenness to learn.
