Conduct interviews and assess needs of Client
- Conduct necessary case work interventions
- Document and evaluate case outcomes
- Prepare reports for administrative investigation and intervention purposes
- Provide Information and Referral services to link Clients to appropriate social services
- Maintain proper records of cases and provide regular and timely statistics of Clients and their progress
- Liaison with and develop networks with community partners and other forms of community support
- Engage clients in a professional and therapeutic manner to understand needs, and to build rapport with clients including all members of the family
- Plan, implement, monitor and evaluate therapeutic group work for specific individuals or target groups of clients

**_Technical Competencies: _**
- Degree in Social Work or post-graduate Diploma in Social Work from an accredited institution
- At least 2 years of case management experiences is preferred. Fresh grad is welcomed to apply.

**_Core Competencies: _**
- Strong communication skills
- Good report writing skills
- Able to work independently

**_Preferred achievements / characteristics: _**
- Accredited as Registered Social Worker with SASW;
- Willing to work with homeless, destitute and male ex-offender;
