SENIOR MEDICAL SOCIAL WORKER

Central location; public healthcare cluster, Care & Counselling

Our client is one of three public healthcare clusters and an integrated general hospital in Singapore that offers holistic care from acute, sub-acute to rehabilitative settings, and reduces the need for a patient to transfer to another healthcare institution. 

Overview:

•    To provide supervisory support and clinical social work intervention in the areas of counselling and case management, crisis intervention and discharge planning for continuing care.

•    To provide psycho-emotional, socio-economic input to the medical and multi-disciplinary health care team in order to achieve holistic patient care.

•    To provide clinical supervision to junior Medical Social Workers & Social Work Coordinators. To provide leadership and manage certan administrative responsibilities

Primary Responsibility:

•    Counseling: to provide counselling and emotional support to patients and family members to help them cope with their illnesses and medical treatment.

•    Casework: to work on case assessment and action plan. Preliminary case assessment is to be completed by 2nd to 3rd day of referral. Complete assessment and action plan is to be completed by 3rd to 5th day of referral. Case progress is to be regularly reviewed until patient is discharged home. For patients who we anticipate requiring continue support after discharge from hospital, post-discharge follow up is required until social situation is managed and stabilized.

•    Discharge Planning for continuing care: to provide counselling and support to patients and family members who require special support for care upon discharge from hospital. To provide comprehensive assessment in the psycho-emotional state of patient and family members, their pre-admission coping mechanism, social and financial resources and availability of relevant community resources. To liaise with community to facilitate effective post-discharge support.

•    Crisis Intervention: to provide priority response within same day or by next working day, and intensive case management to patients who go through personal crisis including attempted suicide or are victims of abuse.

•    Financial Assessment and Assistance to patients who require financial aid for medical treatment and living needs.

•    Teamwork with multi-disciplinary team to achieve holistic care for patients

•    Liaison with community organisation for proactive case management.

•    To provide clinical social work supervision support to MSWs & SWCs

Secondary Responsibility:

•    To identify and work on clinical areas for improvement in relation to the assigned portfolio

•    To take on quality improvement projects and activities

•    To contribute learning and sharing in department case conference / journal club / learning prog

•    To provide field supervision to NUS / SUSS undergraduates in Social Work

•    Any other duties as assigned by the department

Requirements:

•    Degree in Social Work/Degree in Arts, major in Social Work/Post Grad Diploma in Social Work/Master in Social Work

•    Effectively bilingual

•    IT knowledge and skill

•    Good interpersonal skills, helpful personality, team player
