Overview
Provide holistic home-based health and social care to enable the frail elderly to age in place and deliver palliative care to those suffering from advanced illnesses
Direct patient care
a Provide independent clinical assessment of patients and formulate individualized care plans within the scope of practice of a Registered Nurse and in consultation with the multi-disciplinary team, referring appropriately for internal or external services as necessary
b Provide guidance to junior nurses on management of patients with complex care needs
c Provide after-hours phone call advice to patients and their caregivers on a scheduled shift-work basis
Competency Development and training
a. Act as an expert resource for nurses and mentor colleagues to aid them in the advancement of nursing practice in providing home-based health care and palliative care in MWS services.
b. Develop and provide orientation / induction programmes and preceptorship for new staff
c. Develop and coordinate regular training for nurses and ensuring regular competency measurement for nurses
Supervision / Manpower Resourcing
a Supervise assistant nurse clinicians/managers and junior nurses in carrying out their responsibilities
b Ensuring adequate coverage for daily operational needs
c Lead and implement strategies and initiatives to promote well-being of nurses
Quality Improvement
a. Develop internal clinical audit frameworks and conduct audits to ensure compliance with nursing standards
b. Lead quality improvement initiatives
c. Assist HOC/Senior Nurse Manager in reviewing and implementing relevant nursing policies and procedures
Any other tasks that may arise as part of MWS service requirements.
Qualifications
Registered with Singapore Nursing Board
Degree in Nursing and/or Specialist/Adv Dip in Palliative Care, Gerontology or equivalent
Working conditions
Home-based visits for patient care within office hours
Working from home after office hours when assigned on-call duties
Office- based work for clinical competencies and audits
