Job Description:

You are to assist the Medical Social Workers in interviewing and assessing patients and their family members for appropriate service referrals. You may be asked to conduct home visits to obtain collateral information and ascertain patient's home physical and familial environment. You are to assist patients to apply for appropriate financial assistance schemes in Singhealth Polyclinics. You are required to perform administrative duties like statistics collation and submission to various regulatory bodies.

Requirements:

  • GCE O or A levels with 6 years of experience in the social service sectors or customer service experience; or
  • Diploma in Social Sciences (Social Work)/ Psychology Studies/ Counselling/ Community Care or Services/ Social Service/ Social Enterprise or its equivalent
  • Able to speak local languages
  • Proficient in Microsoft Office application
  • Good communication skills
