Key Responsibilities

· Assess new clients and develop personalised care plans.

· Perform nursing procedures (wound care, tube/catheter care, stoma care, medication management, monitoring vital signs).

· Conduct health screenings and make referrals when needed.

· Provide health education to clients and caregivers.

· Coordinate care with doctors, allied health and external partners.

· Build rapport with clients and families.

· Support self-management of health conditions.

· Provide tailored health education.

· Case Management & Administration

· Plan and facilitate Homecare services with the team.

· Maintain accurate client records and reports.

· Manage client workload through scheduling.

· Recognize early warning signs of deterioration.

· Provide appropriate emergency or crisis care.

· Handle enquiries about Homecare services.

· Monitor and manage medical supplies and equipment.

· Support training and development of community partners and volunteers.

· Assist in community outreach and partnership activities.

Job Requirements:

· Min Diploma in Nursing and registration with the Singapore Nursing Board.
