The Resident Physician of Home Healthcare Team will manage the clients' acute and chronic medical conditions through regular home visits and telehealth service.
- Provide medical consultation through regular home visits or telehealth service
- Work closely with the multidisciplinary team to develop holistic care plans for clients and optimize their medical conditions and functional status
- Engage clients and/ or next-of-kin to develop a care plan that is aligned with the clients' preferences and goals of care
- Perform bedside procedures such as venipuncture, urinary catheterization, and management of simple tubes and drains
- Titrate and prescribe medication for the clients
- Assist with referral to other internal services eg Home Nursing or Home therapy or external social services if required
- Ensure accurate documentation of medical records
- Provide medical reports, memos, referrals if required
- Attend continuing medical education (CME) teaching sessions and workshops/conferences to keep abreast of medical knowledge
- Provide teaching to Family Medicine residents if required
- Other job-related duties as assigned by supervisor
Requirements:
- Possess a basic medical degree or postgraduate qualification that is registrable with the Singapore Medical Council
- Full registration with the Singapore Medical Council
- In active clinical practice
- Possess relevant experience in home care services would be an advantage
- Able to work independently
- A team player with a passion to care for the elderly
- Possess good interpersonal and communications skills
- Willing to travel to clients' homes to provide services
