As a Transplant Coordinator, you will be responsible for the following duties:

  • Evaluate and manage referrals for live and deceased donor kidney transplants from renal physicians at various hospitals in Singapore (overseas referrals apply only for live donor kidney transplants).
  • Support potential donors and recipients throughout the kidney transplant journey by offering counseling and performing essential assessments for both parties, along with family members considering live donor kidney transplants.
  • Foster strong connections with all relevant stakeholders, including Ministry of Health (MOH) officials, on matters related to organ donation and transplantation.
  • Regularly assess the eligibility of patients on the Kidney Transplant Waiting List, including coordinating lab tests, tracking results, scheduling appointments, and managing follow-up clinic visits.
  • Prepare and assess patients selected for deceased donor kidney transplants, ensuring all necessary preparations are in place.
  • Participate in ward rounds and clinic sessions to review patient progress, assisting with scheduling investigations according to the transplant protocol.
  • Provide discharge instructions to newly transplanted patients and offer support in the follow-up care of complex post-transplant cases.

Requirements:

  • Minimum 2 years of relevant experience in healthcare settings
  • Currently a registered nurse or possess a Bachelor's degree in Psychology; Arts and Social Science majoring in Social Work; or Life Sciences
