As a Locum Pharmacist, you will be responsible for the following duties:

• Actively participate in healthcare teams to manage and treat acute and chronic diseases, ensuring comprehensive patient care.
• Contribute to maintaining and improving patient health by identifying appropriate therapeutic interventions.
• Detect and resolve drug-related problems through diligent medication review and assessment, ensuring optimal therapeutic outcomes.
• Provide evidence-based drug therapy recommendations tailored to individual patient needs and clinical conditions.
• Participate in regular inpatient clinical rounds and ambulatory care clinics to monitor and adjust drug therapies as required.
• Dispense medications accurately, ensuring the correct drug, dosage, and proper administration instructions.
• Offer patient counseling on medication use, side effects, and adherence strategies to ensure safe and effective treatment outcomes.
• Communicate timely and accurate drug information to patients, healthcare professionals, and team members to support informed decision-making.
• Educate the public, patients, students, and healthcare providers on safe medication use and best practices for therapy management.
• Ensure that all drug therapies align with safety standards and support positive clinical outcomes, minimizing potential risks.

Requirements:

• Degree in Pharmacy accredited by the Singapore Pharmacy Council.
• Registered Pharmacist with the Singapore Pharmacy Council
• At least 2 to 3 years' experience in hospital pharmacy
