What We're Looking For

Trillium Health Resources has a career opening for a Senior Manager, TCL Transitional Complex Care to join our TCL Complex Transitional Care team. This position's primary responsibilities include providing input for Complex Care Coordination targeting those with chronic, unresolved, or complex physical, behavioral health, and social determinant needs. The Senior Manager is responsible for oversight of staff, providing proactive intervention care planning and transition to individuals identified to ensure they receive appropriate assessment, discharge/transition planning, and services in the community. This is a mobile position with work done in a variety of locations. 

On a typical day, you might:

  • Ensure that Occupational Therapy Staff and Registered Nurses (RN) follow care plan guidelines regarding OT/Nurse evaluation and assessments.
  • Assess individuals who may have identified needs with behavioral, physical health, co-occurring, co-morbid, or multi-morbid conditions for conditions or issues within the area of occupational therapy that impact transitions from Adult Care Homes to independent living.
  • Oversee collaboration with Care Coordinators/Care Managers, integrated care nurse(s), Occupational Therapist(s), COTA(s), and clinician(s) to contribute to integrated care.
  • Monitor the Care Plan and Transition plan (physical, behavioral health, and social determinant concerns), service delivery, and health and safety of the member.
  • Oversee Completion of occupational and/or nursing assessments and other assessments as needed.

Qualifications

Required:

  • Bachelor's degree with a minimum of two (2) years of documented experience working with the co-occurring of physical health and behavioral/mental health, physical health, or chronic condition population; OR
  • Equivalent combination of education/experience. 
  • Fully licensed by the North Carolina Board of Occupational Therapy as an Occupational Therapist (OT); OR
  • Fully licensed by the North Carolina State Board of Nursing as a Registered Nurse (RN).
  • Must have a valid driver's license.
  • Must reside in North Carolina and within Trillium's Catchment area, which includes the following counties: Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Carteret, Chowan, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, Wilson.
  • Must be able to travel within catchment as required.

Preferred:

  • Advanced degree preferred.
  • Experience working with the BH/MH/SU/IDD population.
  • Experience in supervision of two (2) or more employees.
