Our goal is to provide suitable intervention care for seniors. This is done with a team of trained social workers using a case management approach, monitoring and counselling services. As a Social Worker, you will be required to carry out casework and apply intervention strategies appropriately to improve the well-being of the client and support the families and caregivers in the community.

You will need to

  • Responsible for managing case referrals to support clients' needs
  • Conduct assessment, planning and intervention for cases by engaging client and building rapport, performing needs assessment and advocating for clients' needs
  • Develop and review appropriate intervention strategies in consultation
  • Refer and coordinate care services for clients
  • Provide information on the various assistance schemes or care options available
  • Raising awareness and facilitating advance care plan for seniors
  • Conduct outreach activities to the community

You should

  • Thrive in a collaborative and dynamic environment
  • Have strong communication and stakeholder management skills
  • Be proficient in Google Suite Applications

It would be great if you

  • Have experience in Social Work
  • Are able to converse in local languages

Qualifications

  • Bachelor's Degree preferably in Social Work
  • Minimum 3 years' experience, preferably in community or healthcare settings in Singapore
