With the key focus to expand community-based services, improving the employability of PWIDS, strengthening education, and engendering inclusive community living, MINDSG LTD offers a holistic range of services and programmes across the PWIDs' lifespan.
**RESPONSIBILITIES**
The Programme Executive contributes actively to the programme needs of MINDS Future Care Planning (FCP) Services under the umbrella of Community-Based Support Services (CSS). CSS is the dynamic and nimble service delivery arm in MINDS that responds to emerging needs of PWIDs and Caregivers in the community. The programme executive will work in synergy with various direct community services to enhance integration and community living of caregivers and PWIDs in the community.
He/she will be involved in the planning, coordinating and executing of programme events and activities. He/she will contribute actively to FCP Services and work with team to ensure the efficient running of FCP programmes which includes working with caregivers on planning for the future and life planning. He/she will also assist the team in promoting and enhancing MINDS online future planning website (FutureReady by MINDS).
A proactive thinker, resourceful and responsible team player, he/she works closely with team to raise awareness of future care planning matters, plan programmes and ensure smooth delivery of programme. He/she will also connect and coordinate with other organisations to execute collaborative, inclusive and client-centric programmes, to manage communications and feedback from stakeholders, and to conduct outreach activities to other organisations and the community.
**Development of Life Plans and Caregivers Engagement**
- To attend to queries and referrals on CSS programmes, in particular on future care planning
- To support team in development /reviewing of client's individual life plans
- To support in conducting needs assessment to screen clients of suitability
**Programme and Event Coordination on future care planning**
- Engage in long term care planning conversations with caregivers.
- Promote FCP and Futureready toolkit via social media
- Content creation and/or organising of publicity materials for distribution to key stakeholders
- Plan and organise outreach activities workshops /support groups on future care planning to client group/s, other social service professionals/ organisations and community
**Build Stakeholder Relationship**
- Manage communications and feedback from caregivers and stakeholders
- Coordinate with internal and external agencies for the execution of events and/or activities
- Develop and support outreach activities to client group/s, other organisations and the community
- Lead in Volunteer / Befriending management for FCP programs
**Resource Management and Administrative Support**
- Maintain and track records and progress of assigned events and/or activities
- Maintain and regularly review text content of Futureready website and update information to keep abreast with current best practices.
- Maintain and upkeep mailing lists and database for proper reporting purposes
**QUALIFICATIONS**
- Degree holder in any disciple
- Certificate in basic case management will be an advantage
**OTHER INFORMATION**
- Social service / programme management experience will be an advantage
- Hands on with strong communication and good interpersonal skills with an ability to influence
- Well organised, attention to details
- Have strong Interest and open attitude towards understanding Persons with Intellectual Disability and their caregiver needs.
- Relevant Digital literacy and mass communications background
