- Coordinate and monitor delivery of services for seniors.
- Conduct home visits to ensure clients’ compliance to care plan.
- Arrange and conduct initial assessment with social workers.
- Maintain and update case notes and records of clients.
- Refer and coordinate care services for clients to ensure they are adequately supported to continue living in the community.
- Participate in clinical supervision and case discussions.

**Group Work**
- Assist social worker to organize and co-ordinate groupwork.
- Support in groupwork facilitation and evaluation.

**Community Work**
- Provide information and referral to appropriate services.
- Assist in community outreach.
