MINDS has been caring for Persons with Intellectual Disabilities (PWIDs) and their families since 1962.

Grounded in the belief that all PWIDs have innate abilities and talents, MINDS is committed to empowering them and enhancing their quality of life, while advocating for greater inclusivity for PWIDs as a nation.

MINDS is today one of the largest charities in Singapore employing more than 850 staff. With the key focus to expand community-based services, improving the employability of PWIDS, strengthening education, and engendering inclusive community living, MINDS offers a holistic range of services and programmes across the PWIDs’ lifespan.

RESPONSIBILITIES

Life Coaches provides Intervention and assistance and enhance clients community living and daily independent living skills according to future care plans developed that was discussed with caregivers and social service and/or healthcare professionals under Community Based Support Services (CSS).

CSS is the dynamic and nimble service delivery arm in MINDS that response to emerging needs of PWIDs and Caregivers in the community. Examples of services include Me Too Club, Homebased Care Services, Caregivers Support Services, Special Student Care Centres and Appropriate Adult Services to enhance integration and community living of caregivers and PWIDs in the community.

The Life Coaches maintains up-to-date documentation of clients' conditions and learning progress and also provides inputs for the development and review of future care plans. He/she ensures house rules are followed by clients and recommends measures to mitigate risk to the clients. He/She conducts activities for clients to promote independence, health, wellness, and quality of life and provides guidance to clients on personal and social skills, independent living, digital and work skills.

A caring, patient and responsible professional, the Life Coach works with various communities stakeholders and partners to advocate for the needs for clients to continue to stay independently in the community.

**Provide client care**
- Provide training and support to clients in their daily living
- Support social service and/or healthcare professionals in providing assistance to clients in managing their healthcare needs
- Perform activities according to future care plans
- Provide inputs for development and review of care plans
- Maintain up-to-date documentation of clients' conditions and care treatments
- Guide and teach in navigating resources and travelling to their appointments and activities to ensure safe transfer.
- Provide clients with information on social support services available at other organisations
- Maintain order, safety and cleanliness of the care environment
- Respond to unusual or challenging behaviours of clients
- Recommend measures to mitigate risk to clients.
- Follow advisories with regards to basic health, hygiene, and safety to provide a safe and healthy physical environment.
- Ensure Training Materials is maintained in a clean and working condition.
- Adopt proper documentation of incident reports

**Conduct programmes and activities for clients**
- Conduct social, recreational and educational activities to promote independence, health, wellness, and quality of life
- Provide input for the design of social, recreational and educational activities to promote independence, health, wellness, and quality of life
- Guide clients in the development of basic personal and social skills, simple academics or work skills
- Identify areas of improvements in programmes, activities and trainings

QUALIFICATIONS
- Minimum N Levels/ ITE

OTHER INFORMATION
- Added advantage for those with experience in education/training of persons with intellectual disability / or multi-disciplinary team environment.
- Self-motivated and fully able to participate in on-site and off-site activities
- Basic First Aid will be advantage
