**Integrated Care Planning**
- Mobilise cluster-wide support through GIC/NHG Clinical Board for the development of new care programmes in key priority tracks identified for population health impact.
- Oversee regular review and evaluation of funded Population Health and clinical programmes. Seek inputs from NHG Clinical Board to review, curate and coordinate development of new key care programmes identified by ICOs.
- Collaborate with external stakeholders to share and spread NHG’s learning points in Population Health where applicable.
- Utilise useful metrics and analytics to gain insights into trends, patterns and outcomes of programmes to provide periodic reporting to GIC/Clinical Board.
- Oversee development of clear measures of success and accountabilities for ICOs to drive improvement of cluster results.
- Support the development of dashboard for ACO performance reporting and accountability.

**Population Health Funding Management**
- Oversee funding management of MOH-funded projects. Working with internal NHG stakeholders to ensure timely and accurate claims and workload submissions and reimbursements.
- Work with HQ Group Finance to sustainably seed-fund population health projects via the Enhanced Population Health Fund, oversee grant evaluation and selection, claim management fund disbursement, and outcome reporting of these EPHF-funded projects.
- Support and report spread, adoption and mainstreaming across NHG institutions and zones for care integration.
- Oversee continued funding of specific priority programmes as directed and resourced by GIC EXCO.

**Team and Operational Leadership**
- Provide direct and indirect operational leadership and oversight to a team. Oversee the design and development of success measures and indicators to evaluate team performance.
- Staff development: ensuring relevant training of staff to build skillsets and learning related to the job function, as well as foster development for future roles, including participation in relevant meetings, committees & taskforces.

Any other duties and services that Reporting Officer assigned.

**Job Requirements**
- Minimum degree in healthcare or relevant disciplines.
- Preferably with 8 years of relevant working experience in healthcare sector.
- Experienced and with a proven track record of driving system level change, health system integration and care transformation.
- Ability to function effectively as a change agent and lead change.
- Must possess strong administrative, budget management and project coordination skills.
- Strong written, analytical and communication skills.
- Strong presentation & public speaking skills.
- Ability to establish rapport & work closely with healthcare sector leaders.
- Able to manage change & deliver project outcomes against a rapid-evolving macro-environment.
