Job Summary
The Renal Coordinator is responsible for the administrative of the Combat Kidney Programme, which aims to provide screening, early detection and disease specific case management for patients with early Chronic Kidney Disease (CKD) Stage 1 to 3A.

Renal Coordinator will also counsel patients on lifestyle interventions and risk factors control to prevent progression of CKD to end stage kidney disease. He/She will help to coordinate care with GPs for patients with CKD stages 1 to 3A.

Job Responsibilities
Liaise with GPs to identify patients and facilitate albuminuria screening for early detection of CKD.
Arrange for appointment for patient with albuminuria from GP for review by the coordinator.
Review patients with albuminuria referred from GP, arrange for needed additional investigations and liaise with managing nephrologists regarding subsquent management and review.
Maintaining and updating database for CKD project.
Assure that appropriate and complete documentation is performed and maintained, and provide monthly statistics reporting.
Plan for outpatient visit as per available clinic slot and estimated urgency to be seen.
Conducts education and counselling for patients and their families/carers with regards to lifestyle interventions and risk factor control to prevent progression of CKD as well as education on complications of CKD.
Liaise and follow-up with GPs on CKD patients, virtually tracking patient's laboratory result on a regular basis and refer back to nephrologists should results show abnormality.
Prepare to undertake any suitable tasks as assigned.
Job Requirements
Polytechnic Diploma in Social Sciences, Psychology, Counselling, Social Work or Nursing recognised by Singapore Nursing Board.
Fresh graduates are welcome to apply.
Some experience in the healthcare industry would be an advantage.
Ability to speak different language/dialects would be advantage.
Proficient in MS Office Applications and technically inclined (i.e. ability to pick up computer or technical skills efficiently)
Good interpersonal and communication skills; ability to work in teams.
