
Clinical Pastoral Care Counsellor

**MAJOR DUTIES AND RESPONSIBILITIES**
**(A) SPECIFIC (80%)**
1. Conduct effective spiritual needs assessments and interventions.
- Identify pastoral health needs and impact upon patient’s health.
- Identify strength, hopes and coping strategies.
- Develop intervention based on assessment in partnership with patient/family members and members of the multidisciplinary healthcare team.
- Monitor effectiveness of intervention and provide feedback to all stakeholders, provide guidance and adjustment accordingly to achieve desired outcome.
2. Assist healthcare team members to develop a deeper appreciation of spiritual health
needs and impact on care delivery through
- educational programs
- sharing at multidisciplinary team meetings
- debrief and bereavement period after demise of patient
3. Conduct of Advance Care Planning (ACP) discussion and participate in ACP development programs in YCH.
**(B) GENERAL (20%)**
- Collaborate with the inter-disciplinary committees in the development of pastoral care program and /or interdisciplinary quality improvement projects.
- Propose, redesign and implement strategies with the medical and allied health team to address opportunities to improve clinical service.
- Function as a clinical resource person to all members of the health care team in ACP palliative care and pastoral care service.
- Support and communicate the hospital organizational structure, philosophy objectives, goals, policies and procedures to employees, patients, family, community and the medical staff.
- Assume additional responsibilities or tasks as assigned by Reporting Officer.
- Participate in development of pastoral care and ACP programs in YCH throug education and research.
**JOB REQUIREMENTS**
**EDUCATION, TRAINING AND EXPERIENCE**
- Locally recognized certificate in pastoral / spiritual care would be an asset.
- Previous work experience in provision of pastoral care in healthcare industry
- Effective communicator with a pleasant personality and inter-personal skills.
- Possess positive work attitude
