- Provide therapy and care support for clients in the senior care centre with instruction from Primary Therapist, under the supervision of centre manager ensuring:

- Dignity of client
- Client’s safety
- Confidentiality of the client’s information
- Advocacy on behalf of the clients
- Perform vital signs assessment and, if necessary, pain score prior and/or post exercises or activities
- Assist therapist in guiding clients and their next-of-kin/ caregiver on:

- ADL
- Fall prevention
- Home safety
- Transfer techniques and devices
- Exercises
- Mobility Aid
- Wheelchair mobility (indoor and outdoor)
- Conduct maintenance activities which includes physical, recreational and other activities planned by the Therapist or Centre Manager.
- Work with care staff to surface issues at the Multi-Disciplinary team to meet the needs of the respective patients.
- Monitor client’s response to treatment and report red flag issues and change in mental status, side effects of medication, unusual incidents which impede on safety to staff-in-charge in a timely manner.
- Coordinate client’s visit schedule using the weekly planner. To inform clients/caregivers on visitation schedule including changes in a timely manner.
- Perform transfers, and assist in mobility in a safe manner.
- Perform proper documentation of patients’ care/progress notes in a timely and accurate manner.
- Maintain Inventory for supplies and equipment.
- Simple equipment maintenance.
- Roster for shift work such as for escort duty when necessary.
- Any other duties as assigned which include administrative tasks, data collection etc.

**Job Requirements**
- GCE O or N level
- WSQ Higher Certificate in Healthcare Support (Therapy Support), Certificate in Therapy Services or equivalent
- Physical fitness to lift and manoeuvre patients during treatments and exercise for extended periods of time
- Computer literate to do electronic entry into electronic medical record.
- Experience in healthcare/community care setting would be an advantage.
