**RESPONSIBILITIES**

**About the Job**

The Social Worker is responsible with working with clients and their family in the day centre(s) to provide psychosocial support and interventions for persons receiving centre and home-based services.

**Key Responsibilities**
- Work closely with clients and family with regards to their admissions and discharges to the day centre services.
- Assess client's support system/network and household condition as part of assessment and care planning.
- Provide case management services and facilitate appropriate casework interventions.
- Work closely with multidisciplinary team and other healthcare professional to discuss and implement holistic care plans for clients.
- Liaise and make appropriate referrals to community service providers to meet the needs of the clients and their families.
- Maintain accurate and current documentation of casework.

**QUALIFICATIONS**
Degree/Graduate Diploma in Social Work
- Preferably at least 3 years of experience as Social Worker in social service sector.
- Experience and knowledge in the healthcare/eldercare/community care sector is a plus.
- Good interpersonal and communication skills with persons of all levels.
- Ability to work independently as well as being a team player.

**Working Hours**

Under normal circumstances the hours of duty will be as follows:
8.30am to 5.45pm (Monday to Friday, 45 minutes Break inclusive)
