The Pharmacist will report to the Principal Pharmacist.
- Review and process drug orders from the doctors.
- Dispense medications to patients.
- Provide counselling service to patients about their medications.
- Responsible for overseeing suitability of medications to be stocked in consultation with the Medical Director. Develop an alternative drug list in event of drug shortages and ensure emergency kits are stocked and adequate.
- Monitor drug usage trends, perform audits and drug utilisation reviews and make recommendations.
- Oversee and perform or direct the performance inspections of drug storage areas within the Hospice.
- Responsible for ensuring safe and effective medication use within the Hopisce.
- Lead medication safety initiatives and activities. Help to identify, resolve and prevent medication-related problems and ensure accurate and timely distribution of pharmaceutical items.
- Perform procurement, supply and distribution of controlled drugs.
- Participate in regular inpatient and home care clinical rounds with the multi-disciplinary team.
- Ensure all procedures are in compliance with national and legal requirements and the hospice’s policies and standard operating procedures.
- Provide support for the development of drug information resources for healthcare professionals.
- Provide medication related training for pharmacy team, doctors, nurses and other healthcare workers as relevant.
- Involve in quality improvement and patient and medication safety activities within the Hospice.
- Supervise and guide pharmacy technician or assistant in their performance of duties.
- Participate in pharmaceutical and clinical research efforts in clinical areas as relevant.
- Make recommendations for changing of medication use policies and procedures for relevancy to practice, quality and safety considerations and compliance with national, legal or institution requirements.

**Requirements**:

- Bachelor or Masters degree in Pharmacy.
- Full registration with Singapore Pharmacy Council.
- Minimum 2 years of clinical practice experience.
- Able to work independently and as a team.
