**Health Coaching Responsibilities**
- Prepare and organize coaching materials in advance to ensure a smooth and effective coaching session. Adhere to health coaching materials when dispensing advice.
- Administer health and lifestyle questionnaires, take biometric measurements, blood pressure and pulse readings depending on the specific requirements of the health coaching session.
- Ensure questionnaire results, measurements and readings are recorded immediately in digital format to minimise errors and lapses.
- Effectively communicate and interpret health screening results to participants.
- Create holistic health action plans in partnership with participants
- Review and analyse participants' current and past health metrics, reports, and coaching notes to develop/tweak personalized coaching plans to guide participant to continue improving their health.
- Employ appropriate/relevant counselling techniques to motivate participants to make changes towards a healthier lifestyle.
- Nudge participants towards relevant avenues or habits to facilitate adoption of healthier lifestyle behaviours (e.g., sign up for HPB-run/ActiveSG/physical activities/classes/ workshops) and assist them in signing up for such activities.
- Confirm next appointment with participant as required and follow-up with them accordingly. Ensure virtual coaching sessions (phone-call or video call) are recorded for audit and verification purpose.
- Perform other duties as assigned by the Project Lead or Management.
