**Roles and Responsibilities**

Reporting to the Group Supervisor, the Social Worker/ Case Manager works closely with the team to serve clients and caregivers.

**Casework - Engagement & Assessment**
- Conduct comprehensive assessment and evaluation of the client’s health and social status.
- Prepare a proper plan of care for the client and client’s support system.
- Coordinate and manage services to meet the client’s individual health and social care needs.
- Coordinate multiple aspects of care with client/client’s support system and health care providers.
- Educate and advocate for the client on necessary services and present treatment options, gather information about community resources, clarify insurance benefits, and provide any needed health education for timely and informed decisions
- Ensure continuity of care by assisting the client’s transition to the next appropriate level of care or provider
- Encourage the appropriate use of health care services to improve quality of care and maintain cost effectiveness.
- Monitor the plan of care and if needed, intervene to achieve desired care goals and outcomes for both the client/client’s support system and the health and social care provider.
- Assess and address psychosocial needs of the client, client’s support system, environment.
- Identify practice improvements and use evidence-based knowledge to inform practice.

**Casework - Case Review & Documentations**
- Follow up and review care/ case plans and provide recommendations to modify care/ case plans where necessary.
- Evaluate the process as well as the outcome objectively and recommend possible ways for future improvement.
- Document and maintain case files and prepare reports including company electronic files according to the required documentation standard.
- Conduct closure of cases upon achieving sustainable service outcomes for clients, families and communities
- Manage case administration through use of company case administration system or InterRAI

**Casework - Goal Setting & Intervention Plans**
- Analyze and evaluate different courses of action in terms of both their immediate and long-term consequences
- Network with other agencies and tap on community resources to provide intervention and support to client
- Participate in intra-case conferences as part of a multi-disciplinary team.
- Assist in the planning, implementation, monitoring and evaluation of group work and provide recommendations for consideration.
- Lead support groups and/or psycho-education groups that aim to provide information to and/ or educate groups of targets audience
- Assist in therapeutic groups that aim to develop target groups of clients
- Implement evaluation of group work through client feedback form, pre/post tests etc
- Prepare group work reports and evaluations

**Community Work**
- Assist in the conduct of environmental scanning and assessment of community needs

through data collection and the conduct of basic analysis (e.g. collation of social data,

conducting interviews or surveys, community mapping, analysis of social trends, etc.)
- Create awareness of AMKFSC services and provide I&R services according to community needs
- Respond to community issues through appropriate interventions.

**Professional Development & Education**
- Participate in developmental programs and supervision for personal and professional growth and development

**Technical Competencies**:

- Degree in Social Work/ Nursing/ Counsellor
- IT literate

**Core Competencies**:

- Good communication and social skills to work with different people - Pleasant personality - Team player, yet independent to carry out most functions - Strong interest in empowering and preserving the dignity of seniors

**Preferred achievements / characteristics**:

- Able to speak dialects will be an advantage
