LentorHealth provides a home medical, home nursing and home personal care service within the home of our Home Care clients.

The Home Care Case Management Executive will support the Home Care Manager in these three services to ensure smooth admission, case management and discharge of clients in these three services.

Location: Macpherson / West Coast Link

**Responsibilities
1. Case Management for Home Care Service
- Provide personalized advice on available eldercare services depending on the client’s needs to ensure right-siting of care
- Liaise with stakeholders and facilitate admission into Home Care Service as required
- Provide financial counseling and assistance to clients and their caregivers to facilitate their admission to Home Care Services
- Support the Home Care Executive to attend to any scheduling issues for Home Care clients

2. Care coordination
- Coordinate referrals as necessary for Home Care Clients to ensure right-siting of care
- Work closely with the medical social worker to ensure coordination of care
- Work closely with multi-disciplinary team to ensure provision of multi-disciplinary care for Home Care clients
- Prepare and coordinate discharge plans for clients to be discharged from Home Care service to ensure continuity of care

3. Assist the Home Care Executive in administrative work for Home Care Service when the Home Care Executive is not available
- Asisst in managing rostering of clients and staff for Home Care Services under the supervision of the Home Care Manager
- Assist to manage claims for Home Care staff
- Assist to manage and upkeep the Home Care equipment and consumables inventory including purchase, stocktaking and allocation of such resources to Home Care staff

4. Stakeholder relationship building
- Build and maintain strong relationships with stakeholders such as clients, caregivers, regional health systems and other key partners so as to further the strategic aims of LentorHealth in the home care sector

**Requirements**:

- Minimum polytechnic diploma, higher education will be an advantage
- Ability to work with Microsoft Word, Excel and other administrative software
- Strong administrative and project management skills
- Good team player with a positive work attitude, strong sense of responsibility and good time management skills
- Service-oriented with good communication skills
