**JOB DESCRIPTION**

The Dietitian works closely with the principal physician and a multi-disciplinary care team to help clients improve their health and wellness, with a focus on nutritional assessment, education and appropriate evidence-based, personalised interventions. This involves both in-person and digital modalities, to support clients in achieving their health and nutrition goals. The Dietitian will also develop and deliver client educational materials, workshops and programmes.

**ROLE**
- Conduct nutritional assessment for clients with different nutritional needs e.g. chronic diseases, gestational diabetes, nutritional deficiencies and weight management
- Develop and provide evidence-based nutritional guidance and meal plans for clients
- Motivate, empower and support clients on nutrition and dietary practices through both in-person visits and digital modalities (e.g. apps) so as to help them achieve their health and nutrition goals
- Respond to client queries, proactively monitor clients’ progress and reach out to clients as needed and appropriate
- Collaborate with care team to promote and integrate high quality person-centered care
- Develop and conduct talks, workshops, group coaching sessions and other education programmes on nutrition-related topics
- Develop nutrition-related educational collaterals
- Plan and manage nutrition-related programmes for different client populations

**REQUIREMENTS**
- Masters / Bachelor’s degree in Dietetics, or Bachelor’s degree in Food and Nutrition with a Postgraduate Diploma in Dietetics
- Accredited by Singapore Nutrition and Dietetics Association or equivalent organisations
- Experienced in providing nutrition-related assessment, counselling and education to adults, taking into consideration the community resources that are available and the individual's context
- Sound clinical knowledge and clinical judgement
- Comfortable with using technology
- Team player
- Able to multi-task and work independently under tight timelines
- Excellent communication skills (verbal and written), and able to tailor messaging to different populations in a culturally sensitive manner
- Strong critical thinking and problem-solving skills
