Job Description:
The incumbent coordinates and implements a care plan for the patients with life limiting illnesses referred to the organisation.
The incumbent must have a strong base of knowledge to act both as a specialist and a generalist nurse.
The incumbent is a role model for other nurses, volunteers and caregivers.
He or She will also be a public educator to raise hospice awareness.
The incumbent works in collaboration with members of an interdisciplinary team in a setting which provides twenty-four hours medical and nursing care, pain and symptom management and compassionate support for their patients and families.
**A. CLINICAL FUNCTIONS**
Exhibits personal commitment and integrity in the care of the patient with advanced disease
Orientates patients and caregivers to the services provided by the organization
Assesses and evaluates patients referred to the organization
Implements and reviews care plans for the patients, working with members of an interdisciplinary team to achieve the highest level of coordinated care for the patient
Monitors pain and other symptom management as well as assesses nutritional needs for patients with advanced disease.
This includes adjusting current medication as needed, so as to achieve optimal symptom control, or formulate appropriate patient-centred care plans utilizing pharmacological and/or non-pharmacological interventions
Identify reversible and non-reversible causes of presenting problems and assess the need for further investigations including the need for hospitalization as appropriate
Identify oncological emergencies and initiate appropriate and immediate management
Conduct family care conferences to address goals of care and manage expectations
Initiate advance care planning discussions
Liaises with other members of the interdisciplinary team in the organisation and with other agencies to coordinate the care given to patient between tertiary institutions, inpatient hospice services and or other ILTC facilities to ensure a seamless transition of care.
Performs nursing procedures as needed to enhance patient comfort
Educates the family in basic nursing skills such as dressings, bathing and skin care as well as the management of special equipment and appliances
Assumes the responsibility of referring patients to other professionals from other domiciliary care agencies for continuing care of catheters, wound management and feeding tubes
Refers patients to in-patient hospices when appropriate
Identifies patients and families in need of financial assistance and refer to MSW
Identifies patients and families in need of specialized counselling or support, and refer to relevant professionals
Prepares family caregivers for approaching death
Provides caregivers bereavement and emotional support when necessary
Performs weekend duty according to roster
Participates in in-service education programmes, seminars and training courses for updating of nursing skills and knowledge in hospice and palliative care as directed by the Director of Nursing or Medical Director
Assumes professional responsibility and accountability for outcomes in nursing practice
Advise on alternative sites of care whenever existing care arrangements are at risk of breaking down
Accept responsibility and accountability for his/her own professional judgement, actions and competence
**B. ADMINISTRATIVE FUNCTIONS**
Performs nursing related administrative duties
Provides regular reports as needed by supervisor
Participates in departmental and organisational activities and events
Deputies Nurse Manger roles in her/ his absent
**C. TRAINING AND EDUCATION FUNCTIONS**
Initiates and participates in continual learning and development programmes, eg. Nursing seminars and conference, journal club within department and nationally
Supports hospice related training and development programmes
Provide clinical supervision to new or junior nurses in complex case management
Provide mentorship to medical students, bachelor nursing degree students and specialist diploma or advanced diploma nursing students
**D. OTHER FUNCTIONS**
Performs other duties as required to meet the organisations vision, mission statements and the organisation Basics
Custody of Controlled drugs and ensure safe dispensing to patients as prescribed by the doctors
Provide bereavement support together with Medical Social Worker
**QUALIFICATIONS**:
SNB Registered Nurse
Degree in Nursing, preferably a Masters in Palliative Care.
Post registration experience in oncology, Geriatric Care or Palliative Care, General Medical & Surgical Care, Intensive Care nursing.
Daily travelling is required
Monday to Thursday 830to 530pm
Friday 830 to 5pm
Salary up to $5500
