- Provide care coordination and service linkage to clients with or who are at risk of mental health issues and/or dementia, with the appropriate resources and support including caregiver support.
- Plan and conduct outreach to the community and stakeholders including faith-based, corporate and community organisations to promote mental wellness and mental health awareness.
- Plan and conduct targeted mental health and dementia case finding/screening to promote early identification and facilitate timely support.
- Plan and organize structured preventive activities and/or programmes for the community, those at risk of mental health conditions and clients.
- Responsible for maintaining client records that are accurate and timely for submission to AIC for data analysis, in accordance to the CREST programme requirement.
- Keep and maintain client and programme data and documents, in accordance to the CREST programme requirement, as well as the prevailing personal data protection and security guidelines.
- Support in the development of CREST service planning document and manual including set up requirements (infrastructure, manpower, training, recruitment) and processes (outreach, screening, mental health information, service linkage & follow-up).
- Ensure attendance in training and supervision sessions, where relevant, as part of the staff competency required by the CREST programme.
- Participate in relevant meetings with key stakeholders including AIC to share key learning points where required.
- Any other related duties as directed by the said reporting officers.

**Job Requirements**
- Degree in Social Work or a Graduate Diploma in Social Work.
- Registered with Singapore Association of Social Work.
- Conversant in local dialects and/or Malay, Mandarin, Tamil.

**Working Hours**

Monday to Friday: 8am to 5.30pm
