- OtherSupport the development of Advance Care Planning (ACP) so that quality ACP services can be provided to individuals nationally. This is done in collaboration with Ministry of Health (MOH), industry partners and stakeholders and in response to the growing needs of an ageing population.

**Programme Development and Management**:

- Responsible in supporting the development and management of Advance Care Planning (ACP) programme in terms of:

- Developing and enhancement of ACP implementation model in the different care setting
- Supporting the capability building of Service Providers (SPs) who adopted ACP services in their institutions.
- Tracking programme implementation progress in terms of KPI achieve and ensure compliance to timelines in project development and implementation
- Facilitate evaluation to ensure ACP implementation improvement
- Benchmarking with international and local best practices in ACP implementation
- Work with a team and to support the implementation of ACP and monitor its progress during the funded period
- Contribute to the initiatives and activities related to ACP implementation model
- Grant management and monitoring of fund utilization
- Provide secretariat support for ACP related Committees and/or Workgroups.

**Networking and Community Liaison**
- Support in the engagement and collaboration with external and internal stakeholders such as existing and potential service providers to actualise ACP implementation and improvement
- Maintain a network of services, institutions and providers for effective roll out of ACP programmes and dissemination of information.

**Quality**

Participate in continuous learning to keep abreast with knowledge and skills of health and social areas related to care integration and intermediate long term care.

**Job Requirements**:

- Degree in any field
- Prior experience in acute care or community settings and/or a project management role would be advantageous.
- Possesses strong interpersonal and communication skills (written and spoken)
- Knowledge in data analytics and meticulous to details.
