Under the guidance of a Patient Navigator or Community Nurse, you will coordinate and navigate all aspects of patient discharge planning to ensure a seamless transition and continuity of care for patients from the hospital to the community, as well as coordinated care for frail residents in the community.
- You will also be required to:
- Establish strong working relationships with patients, their family, internal and external stakeholders and the community, and be the main point of contact for care coordination and navigation
- Monitor and follow-up on patients’ conditions regularly through phone calls or home visits to facilitate early detection of health issues and interventions, so as to prevent illness deterioration and hospital readmission
- Responsible for educating patients and their family on the self-management of illness and importance of a healthy lifestyle, and for reinforcing individualised patient and caregiver education
