**Job Summary**:
To support, coordinate and assist Physiotherapy department in administrative and management of projects. This includes managing and coordinating with different stakeholders across the continuum of care, data management, and proactively participating in department projects.

**Responsibilities**:
**Project management and coordination**:

- Manage, coordinate and facilitate project independently with different stakeholders
**Case management and care coordination**:

- Manage cases in accordance with established practices in care delivery and coordination of multidisciplinary practices
- Assess, plan, coordinate, implement, and evaluate patient care across continuum of care
- Monitor, track and record variance from standard care protocol
- Recommend and execute recommended strategies to control resource utilisation
**Care management and clinical support**:

- Practice care management and provide clinical support to patients
**Data collection and data management**:

- Data collection, data entry, and perform outcome measurements, report generation and presentation preparation.

**Requirements**:

- Degree in relevant healthcare disciplines (e.g. Nursing, Pharmacy, Psychology, Social Work, TCM) or general degree (e.g. Public Health, Sciences, Humanities and Social Sciences)
- At least 1 - 2 years of working experience, preferably in healthcare setting
- Prior experience in project management and care coordination experience is preferable
- Possess good interpersonal skills
- Ability to speak local languages and dialects
- Basic IT proficiency skills
- Able to hand and maintain complex datasets, and multi-tasked
