Singapore Dementia Singapore Full time

**Responsibilities**
- Conduct case assessment and formulate Individualised Care Plans for carers of persons with dementia.
- Provide intervention such as psychoeducation, supportive counselling and service linkages.
- Liaise and collaborate with multidisciplinary professionals and external service providers for holistic care management.
- Provide case recordings and relevant documentation.
- Participate in team meetings and case conferences as required.
- Assist in the preparation of data and statistics for programme evaluation and reporting purposes.
- Plan and facilitate Caregiver Support and Network and Memories Cafe sessions for caregivers and persons with dementia.
- Liaise and network with external stakeholders for collaboration.
- Support in outreach events and talks.
- Any ad-hoc tasks as and when required.

**Requirements**:

- Preferably with a Degree in an Allied health profession/Social work/ Psychology etc.
- Good oral, written and interpersonal communication skills.
- Enjoys group facilitation and prior experience with running group sessions with persons with dementia or carers will be added advantage.
- Able to work on Saturdays and weekday evenings for scheduled programmes or home visits.
