- OtherTo lead a team in the development of the Eldercare sector:
- Developing and enhancing new models of community-based eldercare
- Building key enablers such as IT system that integrate care and facilitate client transitions
- Identify gaps in the care continuum
- Researching international and other best practices of community-based eldercare
- Tracking project progress and ensure compliance to timeliness in project development and implementation
- Facilitating project evaluation and efforts in sustaining and spreading successful models of care
- Work within a team to support the implementation of the new care model and monitor its progress during the funded period
- Support in the collaboration with internal and external stakeholders such as existing and potential service providers to actualise new models of care with focus on operational needs
- Assist in maintaining a network of services in the community for effective rollout of programmes and dissemination of information
**Job Requirements**:
- Degree in any discipline. Qualification in healthcare related disciplines would be an advantage
- Effective verbal and writing communication skills
- Strong interpersonal skills & ability to establish teamwork.
- Knowledge in geriatrics and community services
- Critical thinker and self-directed
