Since October 2011, **Thye Hua Kwan Moral Charities (**THK**MC)** was incorporated as a charity with an IPC status to provide multiple social and welfare services to the community at large.

**THK**MC provides assistance to the various groups in our community through more than 60 programmes and services for the **elderly and the sick**, **families**, **persons with disabilities**, and **children. **We help anyone in need with full respect to their race, colour, creed, language, culture and religion. DESIGNATION : Staff Nurse (Home Care)

RESPONSIBILITIES
- Conduct care assessment for clients and formulate individual care plan according to client’s needs.
- Conduct home visit for 6 monthly re-assessment of clients.
- Provide nursing service as required, e.g. change of nasogastric feeding tube, urinary catheter change, dressing change, stoma care, monitoring medication compliance and medication packing, parameters/peripheral blood glucose monitoring, etc.
- Discuss with doctor regarding care plan for HM/HN clients if required.
- Assist HH Admin staff by providing advice on medical or nursing issues when required.
- Work as a team with the Programme Executive (PE) Admin staff and Scheduler for client care plan for HPC clients.
- Assist with escalation on nursing care issues for ICS clients.
- Ensure all medical equipment and supplies are in good working order before being used.
- Conduct Care giver Training when assigned.
- Mentor new nurses when assigned.
- Participate in Multidisciplinary Team Management (MDM) meeting with collaborative partners as required.
- Conduct home visit with Social worker when required for clients with social, medical or nursing issues.
- Conduct joint visit with other healthcare partners as required.
- Contribute to admin work as directed by Senior Nurse Manager.
- Perform any ad hoc or additional tasks as assigned by your supervisor or manager.

QUALIFICATIONS
- Degree or diploma-holder in Nursing
- Registered with the Singapore Nursing Board (SNB)
- Trained and has experience in community care and preferably in geriatrics, dementia, and/or end-of-life care
- Willing to travel within Singapore
