**Roles and Responsibilities**
1. Case Management
- Perform needs assessment of community dwelling seniors
- Care plan with relevant stakeholders
- Facilitate timely and cost appropriate service referral and provision to seniors
- Coordinate care with MWS services as well as external providers
- Review care plans on at least 6 monthly intervals
- Participate in case management discussions within MWS and with external service providers
2. Work with the SAC team to design, implement activities and wellness programmes that focuses on building the elderly’s self-help capacity including:
- On-site active ageing programmes
- Organising and participation in community events and outings
3. Administration - assist the Manager
- Design, implement activities and wellness programmes that focuses on building elderly’s self-help capacity including
- Ensure compliance to Finance and Human Resource Policies, maintaining documentation for audits facilitated by Corporate Services
- Prepare data for submission to MOH and AIC which includes but not limited to
- Adaily register of seniors who participate in activities and programmes at the Centre
- Register of Information and Referrals done
- Register of activation of the alarm system
- KPIs of funded programmes
**Qualifications**
- WSQ Diploma in Community & Social Services (Senior Services) or Diploma in Social Service/Psychology/Counselling
- Physically Fit
- Interpersonal communications
- Team Player
- Good documentation skills
- Proficient in Microsoft Office
- Leadership skills
- Weekdays office hours and occasional Saturdays with time off in lieu.
- Home visits
**Job Types**: Full-time, Permanent
**Salary**: $2,850.00 - $4,100.00 per month
Schedule:
- Monday to Friday
- Weekend availability
