DESIGNATION : Programme Associate

RESPONSIBILITIES

Support the work of the Active Ageing Care Hub (AACH) and assist the Supervisor in fulfilling the objectives of the AACH and meeting the targets set by funders and management. The key role of the AACH is to ensure seniors within the service cluster are engaged and supported through:

- Outreach to seniors within the service cluster and profile them according to their needs
- Active ageing programmes according to the needs of the active seniors
- Befriending service for isolated & vulnerable seniors
- Information and referral services

The Associate will assist the Supervisor in the following areas:
**Planning and Operations**
- Implement outreach plans to engage and profile seniors for appropriate follow up actions
- Work with internal and external partners in recruiting and training volunteers to assist with the work of AACH
- Engage, supervise and retain volunteers
- Plan and implement active ageing programs and activities for seniors which are age-friendly and culturally appropriate, and within budget.
- Work with community partners and national bodies, for example, grassroots, social service agencies, NAC, SportSG, etc. on programmes and activities for seniors
- Empower and develop active seniors to serve other vulnerable seniors in the community.
- Refer seniors to appropriate care providers if required and co-ordinate support services for them.
- Collect regular feedback from befrienders on vulnerable seniors’ conditions
- Attend networking sessions as required
- Market the centre and programmes to seniors and community partners

**Administrative Work**
- Assist with data entry, record keeping and report submissions in a prompt and timely fashion
- Support necessary evaluation and all critical reporting processes for the management and funders of the programme
- Handle enquiries about the work of AACH

**Additional Responsibilities**
- Any other duties as assigned.

QUALIFICATIONS
- Mininum GCE ’O’ level
- PC literate and able to use IT systems
- Pleasant, proactive and highly accountable for the work assigned
- Have an interest/passion to work with seniors and community partners
- Possess good interpersonal and communication skills
- Able to work independently and at the same time a team player
- Willing to outreach and engage seniors in the community e.g. seniors’ own home, in the park or coffee shops
- Prepared to work on weekday nights and weekends for events and activities when necessary
