St John’s Home for Elderly Persons is a residential care facility for seniors aged 60 years and above. Since 1958, the Home has been providing quality, holistic and compassionate care services to residents based on their physical, psychosocial, spiritual and emotional needs.
The Case Worker should be a team player with ‘can-do’ attitude, responsible, able to multitask and work independently. He or she works closely with Head of Operations (HOO).
**Objectives of this Role**
The Case Worker is responsible to provide Psychosocial and Emotional Support to residents and their caregivers. The following objectives:
- to assist resident to cope, adapt and thrive with living in an institutionalised environment.
- to promote and encourage harmonious living in the Home.
- to counsel all new residents as part of their adjustment to the Home.
- to follow up with befriending program for all our residents.
- to plan and work with residents and caregivers on their Individualised Care Plan (ICP).
**Main Responsibilities and Duties**
- Assessment of residents with counselling needs to implement suitable intervention programmes (individual or group-based counselling or counselling programs such as befriender programmes, therapeutic group work and activities that encourages harmonious community living).
- Provide counselling to individual resident where required.
- Provide small group counselling or conduct counselling programs where required.
- To work with Caregivers/Sponsors of residents if they need help on residents matters.
- Arrange for visit to hospitals for residents who are hospitalised.
- Review monthly visits of residents by family members and contact family members to encourage visit where necessary.
- Maintain up-to-date individual case file/records for consistent review, follow-up and support to residents being individually counselled.
- Provide reports on the case management and key intervention programs.
- To work and plan Individualised Care Plan with Residents and Caregiver/Sponsors.

such as cup, water bottle, towels, toothbrush, hot water flask, towels, soap etc

**Residents’ Attire**
Check for dirty, worn, torn, loose attire etc

**6-monthly Review of Individual Care Plan (ICP) and Resident’s well-being**
To perform review of 2 to 3 residents per week on their ICP and Resident’s Well Being. This review will be an opportunity to reach out and talk to individual resident in a private setting besides completing the review form. To submit completed forms to Head of Operations (HOO).
**Requirements**:
- Minimum relevant Diploma in Social Service, Gerontology, Psychology, Community Service or equivalent experience of at least 2 years.
- A good team player and supports fellow peers.
- Prompt submission of reports and organized filing of documents.
- Open to ideas and suggest new working methodology.
**Monthly Salary range**: $2,500 to $2,800
