Through a series of early outreach initiatives, Care Corner has evolved into an organisation offering a wide range of programmes and services delivered through various social service centres located island-wide.In a bid to stay relevant to the needs of the community, Care Corner has continuously strived to offer new and innovative programmes to support those in need and the less privileged.

DESIGNATION : Programme Executive

RESPONSIBILITIES
- Lead and supervise a team in running the operations of an Active Ageing Centre, preferably with knowledge of MOH KPIs for SACs.
- Plan, develop and implement elderly Enrichment /Engagement & Empowerment programmes for Active Ageing.
- Attend to enquiries and provide information and referrals to relevant agencies.
- Conduct assessments using Community Screening Tool, plan outreach programs, provide home visitations, and hospital visitations (as needed) for seniors.
- Motivate seniors to join social & other programmes that meet their physical, cognitive, emotional, social and spiritual needs.
- Cultivate strong relationships with community stakeholders, beneficiaries and volunteers to help the SAC thrive in the community.
- Manage administrative duties and prepare KPI submissions to MOH.
- Be part of team to ensure the Centre operates with good governance and stewardship.

**Requirements**:

- At least 3 years of relevant working experience, preferably in an Elder Activity Centre and/or have volunteered previously with the elderly
- Excellent interpersonal and communication skills
- Proficient in IT
- Resourceful, proactive, responsible and client-centric
- Passionate about serving the elderly
- Team player with strong planning and organising skills

QUALIFICATIONS
- ITE/Diploma in relevant disciplines such as Community Care and Social Service, Gerontology, and other relevant fields in Social Science or relevant qualifications and experience in working with older persons or in the social service sector
