Purpose of job:
To deliver eldercare services for clients in active ageing, befriending and buddies programmes at Eldercare centre.

Key Responsibilities:

- Plan, execute and evaluate active ageing, befriending and buddying programmes
- Assist in development and implementation of active ageing, befriending and buddying programmes
- Perform community screening for the seniors (outreach or referred)
- Conducting intake and provide necessary health assessments for the elderly clients
- Conducting befriending and buddying programmes for elderly
- Recruit, train and match volunteers with elderly clients in buddying and befriending programmes.
- Organise and conduct active ageing programme for elderly clients
- Coordinate group work initiatives and outreach activities
- Develop and maintain community partnerships to provide services linkages
- Support social work interventions in community work practice
- Perform administrative work including maintaining up-to-date records of clients and volunteers.
- Any other duties as assigned by Acting Head AAC/Executive Director
