Job Responsibilities:
1) Plan and operationalise Health Precincts and community care team concept to achieve health-social integration with internal and community stakeholders.
2) Develop and track progress of workplans and workstreams on Healthy Precincts.
3) Develop and operationalise plan to empower community to take care of their health.
4) Create community culture to drive behavior towards leading a healthy lifestyle and emphasis on preventive care with community stakeholders.
5) Build partnership with internal and community stakeholders e.g. grassroots organisations, social service agencies and healthcare providers to keep patients and residents healthy and integrated care across care settings.
6) Lead and support the team:
- Ensure department is appropriately resourced to carry out its mission - people, funding, skill & technology
- Develop team members to enhance their skills & harness their potential
- Integrate team with the other RHSO departments & divisions
Job Requirements:
- Bachelor Degree in healthcare or business related field
- At least 7 years working experience, preferably in healthcare and corporate planning/strategic planning and partnership development related areas
- 3 to 5 years of supervisory or leadership experience
- Strategic thinking and planning
- Excellent writing skills
- Strong communication skills
