**Metta Hospice Care (慈光安宁居家护理中心) **is a member of Singapore Hospice Council and Asia Pacific Hospice Network. Set up in June 2000 to offer free home hospice care to the terminally ill of all ages and races, we have since helped more than 5000 patients to live with quality and dignity throughout the last phase of their lives at the comfort of their homes.
We primarily take care of patients who are referred from medical institutions and general practitioners. They are diagnosed with life-limiting illnesses such as cancer and end stage renal failure and usually have a prognosis of less than a year.
We provide a holistic care approach with medical consultations and nursing assessments to our care recipients, as well as psychosocial support to them and their families. Our Social Worker will also support our care recipients and their families on the emotional and social aspects if required.
Our team comprises of two locum doctors, three full-time staff nurses, a social worker, a community engagement officer, two administrative staff, a Medical Care Assistant Manager and a Medical Care Manager.
- To perform home visits to palliative care patients who are being referred
- To perform nursing assessment on the patients.
- To initiate nursing care plan and implement new measures of management.
- To adjust current medication to achieve maximum symptom control under doctor’s instruction
- To work closely with the Medical Director/Outsourced doctors on the cases.
- To educate caregivers/families on basic nursing procedures to ensure comfort and care
- To refer patients to other professional bodies such as inpatient hospice when the need arises for symptom control or respite care.
- To identify patients and families in need of financial assistance and refer to social worker/counsellor.
- To identify patients and families in need of specialized counseling to the relevant professionals.
- To prepare families for approaching death.
- To provide family with the emotional and bereavement support when necessary
**Metta HomeCare (慈光居家护理服务)** started in April 2015 as a pilot programme with Eastern Health Alliance to support the medical and nursing component of care in the community. It is a home-based service that provides a holistic approach of managing patients with chronic illnesses and organ failure.

Our goal is to render early intervention and medical assistance with an individualised care plan to reduce the frequency of re-admission into the hospital. This allows our elderly to age in the comfort of their homes, regardless of race and religion.
- To perform home visits to chronic ill patients who are being referred.
- To perform nursing assessment on the patient.
- To coordinate with doctor for medical assessment and follow-up visits to patient.
- To monitor current medication and compliance as prescribed by the doctor.
- To perform nursing procedures such as wound dressing, insertion of urine catheter, insertion of naso-gastric tube and care of tubings as required.
- To have proper documentation of the cases seen.
- To update the home visits record in the IT system.
- To upkeep the confidentiality of the patients seen and adhere compliance with PDPA.
- To initiate nursing care plan and implement new measures of management.
- To educate caregivers/families on basic nursing procedures to ensure continuation of care at home.
- To coordinate with referring healthcare institution on care management.
- To identify patients and families in need of financial assistance and refer to the Medical Social Worker/Counsellor.
- To identify patients and families in need of psychological support or counseling and refer to the Medical Social Worker/Counsellor.
- To participate in Metta Fund raising activities when the need arises.
