**Job responsibilities**:

- Provide coordination and care management for assigned patients
- Connect patients with community resource and financial assistance
- Perform nursing procedures in home environment
- Develop and evaluate care plan and goals
- Liaise with other healthcare partners who are caring for the same patient
- Participate in multidisciplinary round
- Maintain timely and accurate documentation of patients’ condition and care given
- Provide caregiver training programme and health education to patients / caregivers and evaluate the teaching programmes to meet patients and caregivers needs and expectations
- Participate in quality improvement project and nursing researchJob Requirements
- Registered Nurse with the Singapore Nursing Board
- Minimum 3 years’ experience working in working in acute hospital setting and/or community hospital setting.
- Able to travel for home visits to patients
