- Ensure timely delivery of patient care duties.
- Perform diagnosis and prescribing of medication according to patient conditions.
- Ensure timely follow up with patients conditions.
- Educate the patients and caregivers on the disease condition and treatment progress.
- Ensure up to date clinical knowledge.
- Participate in outreach and health promotion program.
- Other ad-hoc duties.

**Requirements**:

- With MRCP (UK) Diploma
- Existing registration with Singapore Medical Council or basic medical degree from recognised universities
- Be in active clinical practice for the past 2-3 years
- Postgraduate training in geriatric medicine, palliative medicine and/or family medicine is preferred
- Conversational in English, Tamil or Hindi
