**Home Care Nurse x 3**

**Office hours, alt sat half day.
You shall provide holistic nursing support to the clients and their caregivers at home, to help them to keep well in the community and minimize need for admission to the hospital.
- Conduct screening of Home Nursing referrals
- Conduct assessment of clients in their home environment and develop an individual care plan for clients
- Perform basic nursing procedures such as change of NG tube, wound dressing, catheterization, medication packing etc.
- Work with a multidisciplinary team and co-manage clients with other service providers, if required
- Provide appropriate health education
- Provide caregiver training and support the caregiver in navigating the eldercare services, including their own psychosocial needs
- Assist with administrative work (e.g data entry, review and development of SOP)
- Perform other duties as assigned Requirements
- Diploma/Bachelor's Degree in Nursing or equivalent
- Registered with the Singapore Nursing Board
- Able to peform basic nursing procedures independently
- Experience in Home Care service is an advantage
- Enjoy travelling to multiple places for work
- Good communication skills
- A team player as well as an independent worker
- Office hours. Requires to work 5.5 days - alternate Saturday.
