The role of the Senior Care Coordinator is to facilitate and expedite discharges to community hospitals, including assessment of patient’s discharge plans and communicating to patients/family.

Close monitoring of cases is required to ensure patient’s smooth and fast discharge to the community hospitals.

Reporting to the Section head of Outflow team, Inpatient Ops, the role of a team lead in CHRT is also to foster collaboration, provide guidance and support for the team members.

**Responsibilities and Duties**

You will be responsible for the following:

- Provide supervisory, guidance and foster collaboration among team members.
- Perform reporting officer duties such as developing career progression pathway for team members and competency assessment of the team.
- Carry out discharge plan assessment for inflight cases.
- Provide patients and their family members with relevant and timely information and assess their needs when choosing a suitable Community Hospital.
- Document the assessment, care plans, and outcomes timely and accurately in the appropriate IT systems.
- Monitor the referral trends and identify service gaps and any transfer delays.
- Establish and maintain partnerships between Hospital and Community Hospital referral teams.
- Perform any other duties as assigned by Head Ops & Admin

Job Requirements:
**Qualification**:

- Degree in Health Science/Nursing.

**Experience**:

- Relevant experience with minimum 5 years in the supervisory role.
- Possess good supervisory and leadership qualities.
- Good verbal and communication skills.
- A good team player who is able to work efficiently in a multi-disciplinary team.
