Physician Case Manager singaporean

**仕事内容**:
- Manage and follow up cases which are referred by Teladoc Health
- Call the patients to collect full medical records, understand their medical status and requests
- Provide comprehensive health advice based on the patient’s situation through call
- Help gather medical records/documents which are needed to be reviewed by experts
- Complete clinical case summary by following the company’s standard
- Explain expert’s opinion report to the patient
- Fulfil the above task requirements based on the standard turnaround time per flow
- Cooperate with Medical director and care coordinator improve service quality and customer satisfaction
- Deliver remote advisory service as requested and committed by Advance Medical, including but not limited to Expert Medical Opinion (EMO), Health Advisory, Chronic Disease Care, Coaching/Education, and Referral.
- Cooperate with Medical director and care coordinator improve service quality and customer satisfaction
**Qualifications**
- Medical Degree from a reputable university
- At least 10 years of clinical practice experience and still practice actively currently
- Fluency in English. Mandarin would be a plus, but not required.
