As the Head of Psycho-Social Care, you will provide visionary leadership in reshaping the care framework for persons with dementia and mild cognitive impairment (PMCI). You will drive the integration of evidence-based practices and develop sustainable, person-centred care models. This role involves working with multi-disciplinary teams, community partners, and caregivers to create an environment where dignity, wellness, and quality of life are at the forefront.Key Responsibilities:Leadership & Strategy: Lead the strategic direction of the psycho-social care team, setting benchmarks and developing new care streams for PWD and PMCI in the community and the Lodge.Innovative Care Models: Synergize PCC with the Ability-Centered Care (ACC) model and mindfulness-based approaches, creating scalable, sustainable care solutions for evolving profiles of PWD and PMCI.Community Engagement: Engage and empower caregivers and community partners to co-create innovative care solutions, expanding services beyond Singapore through collaboration and knowledge exchange.Human Capital Development: Build and grow a high-performing, compassionate team that embodies AHL’s values of respect, responsibility, and resilience.Quality Assurance: Develop robust systems for continuous improvement in care delivery and ensure compliance with health, safety, and regulatory requirements.New Business Streams: Lead efforts to create new, sustainable service models that meet the needs of diverse client profiles while ensuring affordability and scalability.Value PropositionAt Apex Harmony Lodge, we are committed to providing an enriching, challenging, and supportive work environment.
