As a Care Coordinator, you will play a pivotal role in supporting clients on their journey toward improved health and wellness. You will work closely with individuals to provide personalised guidance, coordinate care, and foster positive behavior change through various avenues such as digital health platforms and Healthier SG initiatives. The role requires strong communication skills, deep understanding of health and wellness principles, and a passion for helping others achieve their health and wellness goals.
Key Responsibilities:
•Provide individualised support and care coordination for clients and/or participants that are aligned with lifestyle goal(s) set by General Practitioner (GP), utilising digital tools and resources.
•Facilitate health and wellness coaching sessions, leveraging motivational interviewing techniques when applicable.
•Collaborate with healthcare providers and community resources to ensure seamless care transitions and referrals.
•Monitor participant adherence to programmes and encourage continued participation.
•Monitor and evaluate client progress, providing encouragement and adjusting care plans as needed.
•Communicate effectively with clients in local languages or dialects when necessary to foster trust and understanding. Stay updated on the latest health and wellness practices and integrate them into client care.
•Any other ad-hoc duties assigned when appropriate
Job Requirements:
•Professional qualifications in Nursing, Social Work, Allied Health, Health Services Management, Health Management & Promotion, or a related field.
•Certification in Motivational Interviewing and/or Health Coaching will be advantageous.
•1–3 years of experience in healthcare and/or community health settings in Singapore preferred.
•Strong team-player, with natural ability to interact with patients/clients, healthcare team and community partners of all levels.
•Self-motivated and able to work independently
•Strong communication and interpersonal skills. Proficiency in local languages or dialects is an added advantage.
•Willingness to learn and adapt new technology.
•Passion for health and wellness with a strong desire to help others improve their wellbeing.
•Understanding of health and wellness principles and able to apply them in a virtual or in-person setting.
•Good knowledge of various community services and support in the community setting will be an advantage.
