Executive Assistant (Connected Care Specialist)

Hopes Programme
Job Description:
To be part of a dynamic digital mental health service piloting a novel model of care in psychiatry.
Job Responsibilities:
Review eligibility of persons referred into the service.
Onboarding of eligible persons into the digital platform.
Monitor the health status of persons in the service remotely.
Engagement with persons enrolled in the service.
Follow up with patients and escalate to the assigned healthcare worker if needed, as per stipulated care protocol.
Ensure critical information is communicated within the team.
Provide administrative support.
Requirements:
Competency:
Prior experience in mental healthcare and/or digital technology will be an advantage.
A passion for mental health.
Educational and Professional Requirements:
Diploma in any discipline.
Proficient in Microsoft Office applications.
