Job Description : Provide comprehensive case management and supportive counseling for seniors and their family caregivers. Conduct psycho-social and financial assessments to inform personalized care plans and clinical interventions. Plan, develop, and implement community programs and support groups tailored to the needs of seniors and caregivers. Facilitate therapeutic support groups and conduct home and hospital visits as part of ongoing client support. Serve as a key liaison, building and maintaining strong networks with relevant agencies and stakeholders to advocate for client welfare.
