Summary:

  • Working Location: West side - Day Care Centre
  • Salary: $4,000 - $4,500 (based on experience)
  • Benefits: AWS + Bonus + Yearly Increment
Job Responsibilities:
  • Casework management and support with biopsychosocial-spiritual assessment & intervention of patients.
  • Collaborate with multi-disciplinary team to refer patients to suitable the programmes for financial assistance.
  • Participate or assist in developing, conducting and evaluating patients' support groups and community outreach programmes.
  • Deliver psychoeducation to patients, families, staff and members of public through workshops & seminars.
  • Participate in organization/department initiatives and projects.
Requirements:
  • Degree in Social Work
  • Min. 2 years of social work experience in elderly care is preferred
  • Effective clinical skills in assessment, intervention, casework and counselling
