Position Summary Evaluate, design, and implement a patient's plan of care; and participate in local community service. Each Plan of Care is created to increase, maintain or restore function, minimize or prevent disability, and assist patients in reaching their maximum performance levels. Responsibilities Manage and monitor full patient cycle: perform an initial evaluation, establish a treatment plan, progress treatment and goals, communicate to physician, discharge, and follow-up.
